Box 64895, Baton Rouge, LA 70896		 For each model entered, you must include a non-refundable entry fee of $25.00, a portfolio or video of the applicant’s work, and a complete press kit (with bio and photo). All checks and money orders shall be made payable to HIT CITY RECORDS.

A $10.00 LATE FEE is required with any application received after August 1, 2003.
PLEASE READ AND SIGN: I understand that I will not receive any monetary reimbursement and I agree to participate in order that I may receive the benefit of the experience and exposure of LOMF 2000.
